The Role:
This position will include the organisation of support in accordance with various funding bodies such as the NDIA, TAC and NIISQ Participants. Reporting to the Regional Manager and working in partnership with other Support Facilitators, you will supervise the support staff in implementing therapeutic, goal-oriented support services to the clients within your own caseload. This caseload will include participants that have High Intensity Care supports, so a firm knowledge of the health care systems, the ability to manage stakeholder relationships and training of support staff to deliver these supports is a mandatory component of this role. You will be responsible for improved outcomes for participants who access our service.
Your key responsibilities will include:
- The provision of case management support to participants
- Making referrals to services to address participant concerns and coordinate the participant’s care as part of a multidisciplinary team
- Managing a case load through the provision of services in participants’ homes
- Providing support to ensure the service exceeds all accreditation standards
- Maintaining accurate and timely case notes and reporting on participants’ progress
- Providing effective leadership to employees under your supervision
About you:
- Demonstrated knowledge of the National Disability Insurance Scheme and what it means for participants
- Demonstrated organisation skills to effectively manage workload including complex rostering, caseload management ensuring participant satisfaction.
- Ability to work autonomously and as part of a team
- High level of interpersonal skills including the ability to develop and maintain professional relationships within a team, with participants, across service providers, and with various stakeholders
- Professional and compassionate verbal and written communication skills
- Demonstrated experience supporting participants and managing teams for participants who have complex medical needs including, but not limited to spinal injuries, Acquired Brain Injury and Neurological disorders
- High level of problem-solving skills, to successfully identify problems, develop solutions, and implement these using a logical and systematic approach
- Demonstrated competence in computer applications, i.e. Microsoft Word, Excel PowerPoint and Internet Explorer relevant to the role to ensure work is completed in a professional, accurate, and timely manner
Mandatory Requirements
- Qualifications within Disability or Health Care sector. Nursing skills highly desirable
- Current Blue and Yellow Card/NDIS Card
- NDIS Worker Orientation Certificate
- Valid QLD driver’s licence
- First Aid and CPR
- Relevant experience in Case Management is highly regarded but not essential.
