Job description

POSITION OBJECTIVES: 

  • Provide high quality home care services, including monitoring the wellbeing of disabled people, people who need assistance to live independently and their carers to maintain their independence at home and in the community.
  • To comply and exceed the Disability Care Standards by ensuring that clients are treated with dignity and respect, encouraged to make their own choices about their services, receive services in a professional, client focussed manner that promotes a wellness and reablement approach to care in accordance with Disability Care Standards 
  • Model the organisations Values and Behaviours

KEY RESPONSIBILITY AND DUTIES

• Undertake household tasks as assessed and documented in the Service Plan to maintain a safe, secure and hygienic living environment inside the home. 

• Provide personal care support to participants as assessed and documented in the Service Plan. These may include bathing, dressing, grooming, toileting, personal shopping, meal preparation and medication assistance and with prescribed exercise programs per participant care plan.

• Support participants by providing meaningful and enjoyable experiences to a client to promote social connectedness and encourage physical activity as documented in the Service Plan. 

• Undertake a range of other duties that are within the limits of the employee’s skill, competence and training.

ACCOUNTABILITY AND EXTENT OF AUTHORITY 

• Work within the guidelines and established practices and procedures provided and under the supervision of the service delivery manager. This includes adopting and implementing participant centred wellness, health and general wellbeing as per care plan.

• Maintain client privacy and confidentiality. 

• Report all relevant information regarding client’s well-being to the Team Leader.

• Undertake administrative tasks as directed. 

• Undertake a range of other duties that are within the limits of the employee’s skill, competence and training. 

JUDGMENT AND DECISION MAKING 

• Works remotely from direct supervision and closely follows the specific guidelines provided in relation the range and nature of duties to be undertaken. 

• Uses discretion in resolving minor issues that relate to immediate responsibilities and seek guidance and direction from Team Leader for issues not related to day-to-day tasks 

• Demonstrates initiative in understanding required tasks within the home and within available time constraints. 

• Reports to Team Leader any changes in conditions or care arrangements of clients. 

• Acts in a professional manner, demonstrating the following behaviours – positive attitude, adaptable, flexible and lateral/developmental thinking. 

SPECIALIST SKILLS AND KNOWLEDGE 

• Experience and understanding of working with people from CALD (culturally and linguistically diverse) backgrounds, people who identify as LGBTI (lesbian, gay, bisexual, transgender, intersex), people of Aboriginal or Torres Strait Island background, people experiencing financial disadvantage (including people who are homeless or at risk of homelessness), and people with dementia 

• Ability to understand and follow the Participant Service Plan and work with them towards their chosen goals. 

• Ability to meet the inherent physical demands of the job. (Pre-employment functional assessment will be mandatory and ongoing functional assessment reviews may be conducted as required.) 

• Ability to be flexible and responsive to people’s needs. 

• Ability to speak a second language is an advantage.

MANAGEMENT SKILLS 

• Ability to manage one’s own time, set priorities & manage individual jobs to ensure that the work schedule (roster) is completed. 

OCCUPATIONAL HEALTH & SAFETY 

• Take every reasonably practicable step to ensure your health and safety, and the health and safety of all others that may be affected by your work 

• Comply with all relevant legislation and regulation, as well as Hogan Care policies and procedures and reasonable work instructions 

• Ensure that all hazards, unsafe conditions, incidents and near misses are reported promptly and within set timeframes 

• Participate in the improvement of health and safety, including incident investigations and risk management processes 

  • Maintain knowledge of OHS issues relevant to your work INTERPERSONAL SKILLS
  • Ability to work well and communicate effectively with people of all ages and from a wide range of social, cultural and economic backgrounds.

• Well-developed written and verbal communication skills 

• Ability to work independently and within a team and contribute towards team goals 

• Ability to speak a relevant community language would be an advantage

QUALIFICATIONS AND EXPERIENCE 

• Academic (Mandatory): Certificate III or IV in Home and Community Care, Aged Care, Individual Support, Community Services, Cert IV in Ageing Support, Certificate IV in Disability (with personal care module completed), Diploma of Nursing Experience: 

• Experience providing home support services to frail/older people, people with disabilities and people from diverse backgrounds who need assistance to live independently. 

MANDATORY REQUIREMENTS 

• Victorian Driver Licence and VicRoads Licence verification 

• Use of own roadworthy, registered, and insured car during hours of duty (full comprehensive cover)

• First Aid Certificate (HLTAID003) including CPR current within last 12 months 

• Functional Assessment (Pre-employment medical check) 

  • NDIS Worker Screening Check
  • NDIS Worker Orientation Module
  • Police Check
  • Eligibility to work in Australia -if overseas 
  • Working with children’s check
  • COVID-19 Vaccination status or medical exemption
  • 100 Points ID (e.g., passport) 
  • Staff Orientation Checklist
  • Food Handling Certificate
  • Infection Control Certificate
  • Assist Clients with Medication

CHILD-SAFE STANDARDS 

Maintain a child safe culture at the specific Supported Independent Living locations by understanding and activating your role in preventing, detecting, responding and reporting suspicions of child abuse to the relevant authorities by adhering to relevant Hogan care policies and relevant NDIS quality and safeguards commission legislation. 

SECURITY REQUIREMENTS AND PROFESSIONAL OBLIGATIONS 

Pre-employment screening will apply to all appointments. Prior to a formal letter of offer, preferred applicants will be asked to provide: 

• Evidence of mandatory qualifications/registrations/licences, 

• sufficient proof of their right to work in Australia, and sufficient proof of their identity 

• Complete a National Police Check completed via City of Port Phillip’s Provider. 

• Proof of Vaccination Certificate 

• Evidence of a Working with Children Check (employee type with City of Port Phillip registered as the organisation) 

KEY SELECTION CRITERIA 

• Meets mandatory qualifications and requirements of position 

• Demonstrated understanding of the needs of older people, people with disabilities and their carers. 

• Ability to communicate effectively with people of all ages and from a wide range of social, cultural and economic backgrounds.

• Ability to provide feedback on client’s needs, including the ability to monitor and report on the changing needs of clients.

• Demonstrated Occupational Health and Safety Skills including taking responsibility for your own health and safety and that of others in the workplace. 

• Ability to plan work effectively and to work cooperatively as part of a team.
