👉🏼 The need-to-know…

As an ‘Outreach Support Worker’ at Rise, you will support people with lived experience of mental health issues to develop skills and capacity to achieve goals. In this role you will assist individuals on their journey of recovery, through the delivery of a community based, person centred recovery model service. You will respond to and manage crisis in collaboration with relevant adult mental health services and provide regular reports on client’s progress, achievements, and concerns. 

This role is full-time working 76 hours a fortnight, Monday to Friday. You will be required to work in Various Rise locations including but not limited to Helen’s Place in Middle Swan.

👉🏼 The need-to-have…

Here at Rise, we nurture potential, but we do need you to have a few tricks up your sleeve including the following:

  • Certificate IV in Mental Health or similar qualification.
  • Experience working in a Mental Health Recovery model.
  • Demonstrated experience in the development, delivery and assessment of plans that support a person to achieve their goals and live a good life.
  • Ability to use initiative, discernment and judgement while maintaining professional boundaries.
  • Well-developed positive communication skills within a variety of settings (written and verbal).
  • First Aid Certificate (Minimum Basic First Aid).

Appointments to this position will be subject to a NDIS Worker Screening Check / NDIS Worker Orientation Module / Working with Childrens Check / First Aid Certificate / a driving licence and reliable vehicle to use for work purposes.

Costs for new checks will be covered and/or reimbursed by Rise.
