Who are St Pats?
St Patrick’s Community Support Centre (St Patrick’s) is committed to working in partnership with other agencies to support individuals and families who are homeless or at risk of homelessness in obtaining stable accommodation, independence, and improved quality of life. We provide a range of services to support them on this journey, including emergency relief and meals, health and mental health services, drug and alcohol services, life skills training and a range of accommodation options.
Program Overview
Aboriginal Services is a team of Aboriginal people who engage with Aboriginal clients experiencing homelessness or at risk of homelessness and/or accessing our social and support services. In addition to providing basic needs, the centre provides a place where people can participate in social activities in a non-judgmental way and connect with a range of health, welfare, legal and housing and accommodation supports.
Key Responsibilities
- Engage in consistent day and evening street outreach, initially to observe, progressing to self-directed connection, engagement and rapport building with Aboriginal clients sleeping rough
- Assess client’s needs considering their unique requirements and diverse range of challenging factors that may be affecting their lives
- Identify and engage people experiencing rough sleeping, complete VI-SPDAT surveys and upload details to the By-Name List
- Work collaboratively with other service agencies to encourage successful referral paths
- Attend and participate in team, stakeholder and network meetings including program meetings as per program guidelines
- Ensure that the rights, interests and needs of St Pats clients and staff are understood, occur in a trauma informed and culturally safe way, that connection and service responsivity is reflective of the unique language and cultural diversity of the person we support
Selection Criteria
- Aboriginality – This position has been deemed an identified position under Section 50D of the Equal Opportunity Act 1984 (WA)
- Demonstrated experience connecting with clients in an outreach environment
- Demonstrated understanding of existing of existing public and community housing pathways for people in housing crisis
- Relevant experience in delivering services to clients with complex or multiple issues
- Relevant computer skills in MS Office software and applications relevant to the role
Qualifications
- Relevant tertiary qualification or demonstrated experience in similar role and workplace environment
- Previous knowledge and experience collecting data required for specific programs reporting
- Possession of an appropriate class motor vehicle driver’s license
