Who are St Pats?
St Patrick’s Community Support Centre (St Patrick’s) is committed to working in partnership with other agencies to support individuals and families who are homeless or at risk of homelessness in obtaining stable accommodation, independence, and improved quality of life. We provide a range of services to support them on this journey, including emergency relief and meals, health and mental health services, drug and alcohol services, life skills training and a range of accommodation options.
Program Overview
The Mandurah Assertive Outreach Service provides purposeful engagement with highly vulnerable street-present people. Assertive Outreach supports rough sleepers who are disengaged from mainstream services and have multiple complex issues.
Position Intention
The position provides Assertive Outreach and support interventions to persons experiencing rough sleeping and other forms of homelessness in Mandurah, surrounds and on occasion areas of the Perth metropolitan areas. To assist them in accessing support services, accommodation and maintaining housing, improving their quality of life, maximising self-management, independence and participation in the community.
Key Responsibilities
- Provide place based Assertive Outreach providing support to people experiencing rough sleeping
- Work to identify and engage the target groups within Housing First frameworks
- Provide case management of people accessing the service and to connect to long term housing and support
- Deliver activity-based work and/or daily living skills support
- Once housed, providing intensive case management supports to connect people into long-term supports that meet their needs
- Implement a range of intervention strategies
- Maintain accurate statistical data & contribute information for reports as required
About You
- Completion of a relevant qualification (certificate III/IV), and/or relevant work experience (at least 1 year), or Degree qualification
- Ability to engage with individuals within different levels of needs and build and maintain a professional relationship
- Demonstrated understanding and experience working with issues facing people who experience chaotic life circumstances such as homelessness, AOD use, family and domestic violence and mental health issues
- Relevant experience and demonstrated skill in providing support services to people experiencing disadvantage
- Ability to work independently and contribute to the functioning of the team
- A high level of written and verbal interpersonal and effective communication skills
- Current Drivers Licence
- Working with Children Check
- National Police Certificate
- Senior First Aid (or willingness to undertake if required)
Selection criteria
Essential
- Appropriate qualification in relevant areas such as community services/social work/socialscience and/or significant experience working in the homelessness sector.
- Demonstrated understanding of factors contributing to homelessness.
- Experience in the provision of services to people with complex issues including homelessness,disability, mental health, trauma and dual diagnosis.
- Demonstrated understanding of existing of existing public, private and community housingpathways for people in housing crisis.
- Experience working with people of Aboriginal and culturally and linguistically diversebackgrounds.
Desireable
- Appropriate First Aid certificate or willingness to obtain
- Evidence of training and experience in undertaking a trauma informed and client-centredapproach to working with clients.
- Previous knowledge of data collection programs (or similar), such as SHIP, DSS Data exchange
