Summary:
We are seeking a highly organised and detail-oriented Service Delivery Coordinator to join our team. The Service Delivery Coordinator will be responsible for ensuring the timely and efficient delivery of services to our clients, you will play a critical role in planning, coordinating and managing services for individuals with disabilities ensuring they receive the necessary support to lead fulfilling lives.
Responsibilities:
- Develop, implement and monitor individualised support plans
- Advocate for client's needs and ensure they receive appropriate supports and services
- Monitor Service Delivery performance and identify areas for improvement
- Develop and maintain strong relationships with clients and stakeholder groups, conduct regular assessments and reviews of clients progress.
- Provide regular updates to the relevant stakeholders on the status of the clients service requests
- Ensure that all Service Delivery processes are documented and adhered to
- Identify and escalate any issues or concerns to management as needed
Qualifications:
- Minimum 2 years of experience in Service Delivery coordination or a related field (desirable)
- Strong knowledge of disability services
- Excellent communication, interpersonal, organisational and problem-solving skills
- Ability to work independently and as part of a team
- NDIS Yellow Card and current Drivers Licence (mandatory)
- First Aid and CPR - or ability to obtain
- Proficiency in Microsoft Office and other relevant software applications
