About this opportunity
Team Leaders facilitate teams to work in an integrated fashion. Integrated teams work together to ensure that support is provided continuously and seamlessly, including at night, on weekends, on holidays and during periods of leave. To achieve this, Team Leaders support HASI and CLS teams and other services to ensure that participants have access to the supports that they need, when they need them.
An essential part of every Team Leader's role is to ensure the timely and accurate recording of participant information and data. Team Leaders are responsible for ensuring that their teams complete daily, weekly and monthly data entry and maintain file notes, as well as various program documentation such as recovery assessments and plans, risk assessments, referral documentation and more.
Team Leaders are required to travel across the region, including overnight stays where required.
To be successful you will have:
- Min. Certificate III in Community Services and or related relevant experience
- Experience and ability to work with people living with disabilities and challenging behaviours
- Current valid Driver's License
- Satisfactory Police Records Check
- Working with Children Check
- Right to Work within Australia
- NDIS Worker Orientation Module
- NDIS Worker Screening Check
