We are looking for a Team Leader NDIS and Aged Care to join our Port Macquarie team. This position is a fixed term full-time 12 month contract.

As a Team Leader NDIS and Aged Care with Livable, you will focus on:
- Under the direction of General Manager Service Delivery and Growth (GMSDG), oversee the delivery of safe, high quality, and person-centred NDIS and Aged Care services in accordance with relevant legislation, standards, organisational policies and procedures and funding body requirements.
- Develop and maintain strong relationships with participants, families, caregivers, and other stakeholders to enhance collaboration and service delivery outcomes.
- Work closely with the Quality, Safety and Risk Team to manage risk and ensure a commitment to continuous improvement.
- Ensure all restrictive practices and incidents within the area of responsibility are reported immediately, and corrective measures are implemented where applicable.
- Manage SIL vacancies by actively seeking suitable candidates and ensuring vacancies are filled within expected timeframes.
- Motivate, coach, and forge strong relationships with support workers and internal service professionals to ensure their ongoing commitment to the delivery of safe, high quality, and person-centred services.
- Carry out weekly face-to-face support/mentorship meetings with Direct Reports.
- Participate in weekly online or face-to-face support/mentorship meetings with GMSDG.
- Represent Livable at community activities and events.
- Carry out and support on-call processes (via rotation roster)
Your skills and experience will include:
- Advanced knowledge and experience working in the Aged Care/NDIS sector, including a sound understanding of the NDIS/Aged Care Quality Standards, policies, procedures and reporting.
- Demonstrated leadership skills with the ability to inspire and motivate a team.
- The ability to build rapport with participants, families, and stakeholders.
- Sound knowledge of relevant legislation, regulations, and best practices in aged/disability support services.
- Ability to recognise opportunities for process improvement and to propose and subsequently implement approved changes to processes.
- Commitment to continuous improvement and a proactive approach to service delivery.
- Relevant qualifications in the areas of Aged Care, Disability, or Community/Social Services.
To be successful, you will need:
- Working with Children’s check
- National Police check
- Drivers licence [and own insured vehicle if applicable]
- Three mandatory COVID vaccination certificates
You will also be required to complete the NDIS Quality & Safeguards Commission Worker Orientation Module or provide evidence of completion. The successful candidate will also be required to demonstrate entitlement to work in Australia and to disclose any pre-existing injury or disease which may be adversely affected by undertaking the inherent requirements of the position.
