About us
The City of Kingston is approximately 20km south-east from Melbourne's CBD. AccessCare is a City of Kingston service, delivering home care and support services to people who are older or have a disability, as well as their carers and people who are homeless or at risk of homelessness.
The role
AccessCare’s Team Leader Nursing leads the community nursing service provision across our organisation. The role manages a small and dedicated team of community nurses, and is tasked with building strong relationships with service providers and relevant local agencies.
The Team Leader is also a member of AccessCare’s clinical governance team. This team is leading the implementation of the clinical governance standards under the new Aged Care Act and Aged Care Quality Standards as part of the new in-home community aged care program to be rolled out in 2023/24 and 2024/25.
In this role, you will be responsible for:
- Managing and supporting registered nurses to assist clients with general health support, health literacy and providing referrals to other services
- Overseeing the operational requirements of service delivery, quality systems and staff development
- Identifying trends, including wellbeing and reablement, and facilitating continuous improvement initiatives
- Following processes, procedures and quality systems to ensure accountability for funding to government departments
This is a full-time permanent position based in Mentone.
About you
You are a clear communicator with a genuine passion for supporting vulnerable community members. You will have strong interpersonal skills, and have ample experience in managing teams of diverse personalities and helping clients with varied and complex needs.
To be successful in this role, you will also have:
- A division 1 nurse qualification with current registration with APHRA
- Community nursing experience with high level assessment skills
- Experience working with customer relationship management (CRM) software and management of electronic consumer records.
- A strong focus on flexible and responsive service delivery
- An understanding the application of the Aged Care Quality Standards in a community setting
- Advanced ability to sensitively, and at times assertively, communicate with people from all walks of life whilst having understanding and empathy for their complex care needs
Candidates with recent or ongoing clinical experience and experience conducting continence assessments are highly desirable.

The salary for this position starts at $104,653 per annum plus superannuation.
