Role purpose:Reporting to the 'My health for life' Telephone Health Coach Coordinator, the Telephone Health Coach will be part of an allied health team responsible for delivering a state-wide telephone-based risk assessment and behaviour change program. This is aimed to reduce the risk of type 2 diabetes and cardiovascular disease through lifestyle modification. Key responsibilities:Conduct outbound coaching calls to participants enrolled in the My health for life program and flexibility of work hours to cover 8am-8pm.Deliver six sessions to each participant over a six-month period.Utilise motivational interviewing techniques to support enrolled participants to identify key behaviour change goals and implement strategies to improve their overall health and wellbeing.Program scheduling and delivery to a dedicated client load ensuring the ongoing efficiency of the Telephone Health Coaching service.Required Capabilities:A tertiary qualification within health or a similar field.Demonstrated experience in delivering participant education programs utilising a behaviour change approach.Experience in delivering telephone-based Health Coaching interventions or similar health programs directed to individual clients.Demonstrated time and work prioritisation skills.Demonstrated experience in meeting targets within a performance environment.Intermediate (or above) skills in utilising computer-based programs or software requiring accurate data collection, data integrity and timely reportingCommunicate effectively with the team.Additional information:National Police Checks are a mandatory requirement to work at Diabetes Australia, we appreciate your cooperation.Some after hours work may be required for the role.This position is based primarily in Brisbane, some travel may be required.Application closing date: 9am Monday 11 March 2024 Diabetes Australia is proud to be an equal opportunity employer committed to fostering a diverse and inclusive workplace.
