About the Role:
The Toward Home Wellbeing Care Coordinator will be responsible for working collaboratively with individuals experiencing homelessness to link people with appropriate ongoing health services and manage crisis when it occurs. They will be engaged in working with individuals to maintain motivation and engagement to support ongoing access to services through management of complex health and housing needs. Support includes providing an assertive outreach response to people sleeping rough and being part of the Toward Home Resolve assertive outreach roster.
Experience Requirements:
Demonstrated experience in engaging with homelessness community groups and individuals
Demonstrated experience working within a fast-paced environment
Experience in working with Aboriginal communities and the ability to provide culturally appropriate support
Demonstrated experience in working effectively with people with a chronic disease
Ability to capture and share clinical information with relevant health care providers, including in electronic format
Have a good understanding of the local health system including referral pathways
A minimum 3 years’ experience in the area of clinical service delivery
Qualification and Other Requirements:
- Drivers Licence
- Current or willing to obtain a Working with Children's Check (WWCC)
- Tertiary qualifications in Social Work, Social Sciences, Human Services or related
