About the role
Based at our office in North Adelaide you will be part of our dynamic and dedicated Home Care Package (HCP) team. This is a holistic and hands-on role.
You will be responsible for the timely and accurate provision of care coordination, case management and clinical care to clients. You will also be involved in training initiatives and supporting our care staff.
Ideally, you will come with experience in managing commonwealth funded Home Care packages with a strong clinical background…A non-clinical background with a solid HCP experience will also be strongly considered.
To be a successful in this role, we are seeking the following:
- Clinical problem-solving and case management experience in a community setting
- Sound understanding of and demonstrated experience in Home Care Standards Relationship management
- Tertiary qualifications, unrestricted registration with APHRA either as an EN or RN, or qualifications/experience in management in a community care setting (non-clinical)
- Quality assurance and computer literacy with sound knowledge of management information systems
- Strong understanding customer service principles.
- Excellent time management skills and the ability to handle multiple tasks, priorities and deadlines.
- Enthusiastic and service minded individuals who can demonstrate a sound work ethic and attention to detail
- An immediate start is possible for a smooth handover
- Excellent verbal and written communication proficiency, strong interpersonal skills and a continuous improvement approach.
- Actively demonstrated solid understanding of privacy principles, confidentiality and duty of care.
The following checks are also required:
- DHS screenings for NDIS and Working with Children or willingness to apply for these clearances
- National Police Check
- Current full SA Drivers License
- Eligibility to work in Australia
The position is offered from Monday to Friday on a fulltime basis.
