The Role:
An exciting opportunity has to become available to join of our high achieving, client centred team with the Transition Care (TCP)/GEM@ Resi program. The program is seeking a dynamic and experienced Allied Health Assistant Grade 3 to join the team.
There are 2 Permanent Part Time positions available.
Transition Care provides care coordination and low-level therapy from hospital to community care settings, and this role can encompass all streams of Transition Care (Home based and Bed based) and Gem@Resi.
The Transition Care / Gem@Resi team is a friendly and dedicated team who support each other to provide optimal care for aged clients following a hospital stay. Working as part of a multidisciplinary team is required. Supervision and professional development opportunities are provided.
To be successful in this role you will have:
- 5 Years experience working in Allied Health (physiotherapy, occupational therapy) in aged care
- Proven ability to work within a team environment
- Demonstrated ability to follow instructions
- Proven equipment ordering and maintenance experience
- An understanding of the roles and contributions of difference disciplines to client care and discharge planning.
