The Transition Support Teacher at AFL Cape York House for Girls will collaborate with staff from partner schools and the AFL Cape York House for Girls team to provide a range of quality educational interventions designed to support students through their secondary studies. Access theThe Transition Support Services (TSS) Unit provides support to Aboriginal and Torres Strait Islander students, parents/carers, school personnel and the wider school community in the transition from primary to secondary school.

As the Transition Support Teacher (TSS) at AFL Cape York House you will have responsibility for the following:
  • Establish and maintain productive partnerships with relevant partner schools, family and community stakeholders as well as support teams at AFL Cape York House.
  • Collaborate with partner schools, AFL Cape York Staff and the TSS regional office team to case-manage the successful secondary transition of young people applying for boarding placements at AFL Cape York House.
  • Design, implement and coordinate educational interventions to support students' achievement through study in the boarding environment at AFL Cape York House.
  • Lead AFL education mentors to coordinate student support in afternoon study and school-based mentoring.
  • Support the parents/carers of current Year 7-12 AFL Cape York House boarders to connect with their child's school and engage in key decision-making related to their child's secondary schooling.
  • Maintain relevant work records and student data for the purposes of future planning, monitoring, reporting and reflective purposes.
  • Identify opportunities for targeted advocacy for change and take action to support projects that impact on student enrolment, engagement and retention at school and in the residential facility.
  • Participate in relevant and ongoing Professional Learning related to supporting the holistic and academic needs of Aboriginal and Torres Strait Islander students.
  • Participate as directed in school, departmental and AFL Cape York House decision-making processes.
