Transition Support Worker – South East

What you will be doing  

There are three major paediatric hospitals in NSW - John Hunter Children's Hospital in Newcastle and the two Sydney based campuses at Randwick and Westmead which are known as the Sydney Children's Hospital Network (SCHN).

The majority of young people with a complex chronic disease move from these hospitals to a variety of adult hospitals and ambulatory care facilities throughout the state. As increasing numbers of young people with chronic illnesses arising in childhood survive into adult life, many difficulties are encountered during transition from paediatric services. In some cases there is a loss of continuity of care or a fragmentation of care, which reduces quality of life for the young person and their parents / carers.

The Transition Care Network (for young people with a chronic illness/disability) was established in 2004. A Network Manager, three Transition Care Coordinators (TCCs) plus 3 part time support positions were appointed.

The primary purpose of the Transition Support Worker role is to provide support for the Transition Care Coordinator (TCC) to help young people with chronic illness/disability transitioning from paediatric to adult health services throughout NSW and to improve process across the health system.

Remuneration:   $66,027 - $68,085 per annum (pro rata) excluding Superannuation

  1. Experience working with young people with chronic illnesses and disabilities.
  2. Ability to work as an effective member of a team with the capacity to undertake a diverse range of tasks and also to work with minimal direction when required.
  3. Strong communication skills including experience of communicating with young people.
  4. Demonstrated data entry and report writing skills.

COVID-19 Vaccination Compliancy

All NSW Health workers are required to have completed a primary course of a COVID-19 vaccine which has been approved or recognised by the Therapeutics Goods Administration (TGA). Additionally, Category A workers are required to receive a booster dose three months after completing the primary course of COVID-19 vaccinations.  New applicants must have completed the vaccination course prior to commencement with NSW Health, or provide an approved medical contraindication certificate (IM011 immunisation medical exemption form) certifying the worker cannot have any approved COVID-19 vaccines available in NSW.

Acceptable proof of vaccination is the Australian Immunisation Register (AIR) Immunisation History Statement or AIR COVID-19 Digital Certificate. Booster doses are highly recommended for all health care workers who have completed the primary course of COVID-19 vaccinations.
