Our broad range of primary health services, care management and in home and community support services can be tailored to an individual’s needs and with major hubs in Box Hill and Eltham, these services can be delivered locally, in the Eastern and North Eastern suburbs of Melbourne.

The Opportunity
We are looking for an energetic allied health or enrolled nursing professional to join our supportive multidisciplinary team as a Wellbeing Co-ordinator in a part time capacity. The role will undertake work in the Diabetes Connect project.
Responsibilities include but are not limited to;
- Work with clients and the diabetes nurse educator to develop individual care plans
- Connect clients with local services and supports including social prescribing
- Coach patients/families toward successful self-management of their chronic disease.
- Support Diabetes Connect project co-ordinator in GP engagement and reporting requirements
Skills & Experience
- Demonstrate an exceptional level of understanding of service coordination.
- Demonstrated ability to lead, communicate, educate, collaborate and counsel as necessary with particular emphasis given to communication skills and a commitment to collaborating with the rest of the multidisciplinary team.
- Possess a comprehensive understanding of chronic disease self-management and behaviour change.
- Knowledge of, and ability to navigate local health and social support services.
