Upper Hunter Youth Services Inc., located in Muswellbrook, is a community based organisation funded by Department of Communities and Justice servicing the Muswellbrook, Upper Hunter and Singleton Shires. We are currently seeking an experienced and dedicated Youth & Family Support Case Worker to provide support, advocacy and referral to young people and their families ensuring that their wellbeing outcomes are met through the provision of early intervention support services.
Key Responsibilities:
- Conduct client assessments, individual case plans and referrals.
- Develop appropriate intervention programs which support clients with their social context including individual support and group work.
- Liaise with appropriate services, schools, agencies and organisations to promote Service activities through the local area.
- Ensure accurate record-keeping and DEX data integrity, to support effective planning, decision making and contributing to the accountability and continuous improvement of the Service.
- Work within a small team.
- Perform general administrative duties including preparation of Monthly Reports.
- Attend meetings.
Essential Criteria:
- Tertiary qualifications in Social Welfare, Psychology, Certificate IV Youth Work and/or equivalent experience.
- Demonstrated ability in working with young people (10-18 years) and/or their families.
- Demonstrated ability to facilitate group programs and to deliver holistic case management.
- Experience in developing quality case plans or individual case plans.
- An understanding of the issues facing vulnerable young people and their families particularly in our Service area.
- Experience in working with individuals, families and groups with complex needs and from diverse backgrounds.
- Demonstrated ability to consult and network with a range of stakeholders including government departments, schools and community organisations
- Work effectively as a team member and work with minimal supervision.
- Excellent written and verbal communication skills
- Full Current Driver’s License
- Understanding and commitment to Work Health and Safety principles and guidelines
- Computer literacy in Outlook, Word, Excel.
- Current First Aid Certificate or willing to obtain
- Current Working with Children Check and a National Police Check.
