About the role
We are seeking a dedicated person for our Afterhours Youth Case Manager position! This role is responsible for providing intensive and structured support outside of business hours to people on intensive bail or youth control orders.
Responsibilities include
- Establishing effective relationships are established with clients that encourage self-determination and participation
- Providing Case management support to clients in accordance with the TSA model of care and practice
- Development of individual case plans in collaboration with clients and include a range of interventions that address the needs or goals of clients
- Updating accurate client records, and data is maintained for all clients in accordance with TSA standards and procedures using TSA information systems
- Ensuing risks to clients, staff and TSA are identified and bought to the attention of the relevant supervisor or manager as soon as possible
This is a Permanent Part-Time (26 hour) position, based in Shepparton Vic. Salary and conditions are in accordance with SCHADS Level 5.
What you will have
- Tertiary qualification in social work, welfare, community development or related field
- Relevant experience in a social service environment preferably the youth sector
- A current Victorian Drivers Licence
