Youth Worker - Targeted Care Packages
This is a full-time, fixed term opportunity (12 months)

About the Role:
Flexible Funding Packages including Targeted Care Packages (TCP's) are an allocation of funding attached to a specific young person based on assessment of the young person's needs. 
TCP's aim to enable the young person to fulfil their goals and aspirations with flexibility to target responses to their individual circumstances.
TCP's aim to support young people to transition into independent living arrangements or support family reunification.
The TCP Keyworker is responsible for the ongoing implementation and overarching case management responsibility of these packages.

Primary Objectives:
The TCP Key Worker plays a pivotal role in the achievement of outcomes for young people consistent with their Best Interests Case Plan and Cultural Support Planning as they transition from care services towards independence.

Key Worker - Key functions include:

  • Direct engagement and therapeutic support of young people, their families, carers and others to facilitate changes that support achievement of the young person's best interests case plan.
  • Liaise closely with Child Protection, care teams and other professionals to support achievement of the case plan.
  • Primary contact for the young person and their family to collaboratively support them in a holistic approach based on strengths, needs, goals and aspirations.
  • Lead and coordinate effective care teams for young people and their extended familial and professional support networks.
  • Assertive outreach to assist in core functions of support and assessments including risk and safety.

Qualifications & Experience:
Relevant tertiary qualifications in Social Work, Youth Work, Psychology and/or related Behavioural Sciences at degree level with relevant experience or a Diploma with substantial experience in the relevant service stream.
Demonstrated ability to provide a case management and coordination service in complex family circumstances where multiple service providers may be involved, within the child protection system.
