Join the Rehabilitation, Aged and Palliative Care team as a Aboriginal Health Practitioner/Worker.The Division of Rehabilitation, Aged and Palliative Care (Div RAP) provides team-based care for older people, people who are living with disability and people with a life-limiting illness. You can find out more about Div RAP at SALHNThe Aboriginal Health Practitioner/ Worker will work together with multidisciplinary rehabilitation teams to develop a planned approach to engage with Aboriginal people and the community. Engagement, learning and sharing is an expectation for this role across the care continuum. You will work collaboratively with the clinical staff, allied health professionals and other service providers and agencies and actively contribute to the multidisciplinary team processes.Please note: This position is multi-classed as either OPS4 or ASO4 and classification will be determined by qualifications and experience.About the Role:An exciting opportunity exists for a dedicated and culturally sensitive Aboriginal Health Practitioner/worker to join our team. Working within a multi-disciplinary framework, you will play a crucial role in delivering high-quality health clinical care and, social and emotional well-being care to Aboriginal and Torres Strait Islander individuals, their families, and carers. Key responsibilities include:
  • Contributing to effective consultation with staff, wider health teams and the community to gain support and commitment for culturally appropriate health care
  • Supporting and guiding team members on culturally appropriate clinical interventions.
  • Planning, implementing and reviewing appropriate interventions such as referrals, screening and advocacy, assisting in client consultations, participating in case conferences and in shared care planning for clients, and coordinating home visits.
  • Assisting in the provision of client and family consultation, assessment, support, counselling, referral, and follow-up on various health issues.
  • Working as part of a multi-disciplinary team to coordinate the continuity of care and maximise client participation.
  • Participating in quality assurance processes, clinical audits, program reviews, and performance evaluations to ensure service monitoring and continuous improvement.
  • Ensuring client privacy by maintaining security of client records and confidentiality of client-related information.
About you:
  • Registered or eligible for registration:
  • (OPS4) Certificate IV in Aboriginal Primary Health Care- (Practice) and registration with AHPRA as Registered Aboriginal Health Practitioner
  • (ASO4) Certificate III or IV in Aboriginal Primary Health Care (Practice/Community) or equivalent
  • The ability to work appropriately in a culturally respectful way with children, youth, women and their families/carers where there are multiple complexities, diverse cultural backgrounds and expectations of clients.
  • Strong dedication to contributing to quality improvement and best practice service delivery.
