Role Responsibilities
Reporting to the Housing First Rapid Response Team leader this role focus is on engagement with chronic rough sleepers to transition to accommodation (crisis, transitional, mid-term and long-term housing). Reporting to the Housing First Rapid Response Team leader the worker will:
• Provide flexible and responsive outreach services to people experiencing homelessness as required to the identified target group.
• Engage, and build rapport through regular outreach to people who are (or have been) rough sleeping, who are:
- currently accommodated through the crisis responses
- attending engagement hubs; and/or
- in geographical areas identified through the By Name List and other data as priority or hot spot areas
• Work collaboratively with partner organisations rapid response team, in particular support workers to promote effective referral pathways.
• Access client’s requirements taking into account a diverse range of factors that may be affecting them.
• Provide advocacy, support and referrals to clients required to enhance access to essential services and appropriate accommodation.
• Assist clients to develop increased knowledge and life skills, in order to support their reintegration to the community and reduce likelihood of future crisis. (Working with Housing Support Workers)
• Appropriately utilise and manage brokerage funds across the program to ensure services are responsive to client needs and accessing essential needs.
• Work collaboratively to ensure a wraparound and a 24/7 care approach to supporting clients in the program, particularly through Ruah and the after-hours support team
• Listen and respond to ensure culture and practice is blended to support service delivery to meet individual needs of specific groups such as Aboriginal /CALD groups.
• Maintain accurate service delivery and client records, record and maintain statistical data & contribute information for reports as required.
• Actively participate in supervision processes, outreach team meetings and service area meetings.

Experience / Skills Required:
• Demonstrated understanding factors contributing to homelessness.
• Experience in the provision of services to people with complex issues including homelessness, disability, mental health, trauma and dual diagnosis.
• Demonstrated understanding of existing of existing public, private and community housing pathways for people in housing crisis.
• Evidence of training and experience in undertaking a trauma informed and client-centred approach to working with clients.
• Previous knowledge of data collection programs (or similar), such as SHIP, DSS Data exchange.
