About the role:The Activity Support Partner deliver’s person-centred care and activities to participants at our Brookvale day program. You will be implementing participant’s goals to ensure that their activities incorporate what is important for them and what is important to them. You will be providing active support which can include personal care, mobility support and more.
Our main hours are Monday to Friday from 9am to 3pm with the potential to work occasionally up to 6pm, plus weekend shifts from 10:30am-4:30pm, in home care shifts and evening shifts from 6:00pm -10:00pm & 6:30pm-10:30pm.
Duties:
- Interact positively with participants, families, carers and stakeholders.
- Undertale all required aspects of participant’s personal care.
- Make decisions using sound judgement to support and advocate for the participant.
- Accompany participants on excursions or outings.
- Assist the participants to implement their goals.
- Ensure familiarity with the participant’s profile and always carry their emergency plan.
- Hold a valid NSW Driver’s licence and insured vehicle.
- First aid Certificate (or ability to obtain one before commencing).
- Completing a NDIS Worker Check is a requirement of this position.
- Completion of the mandatory NDIS Worker Orientation Module ‘Quality, Safety and You’ is required.
