Organisation Overview
Carbal Medical Services (Carbal) is a Not-For-Profit, charitable organisation that provides health services to the Aboriginal and Torres Strait Islander communities in and around Toowoomba and Warwick.
The core function of Carbal Medical Services is to provide medical services to Aboriginal & Torres Strait Islander people through two medical practices and multiple community programs and initiatives. Carbal covers the regions of Darling Downs, Southern Downs and Goondiwindi, this position will be based in our Toowoomba Clinic.
Position Overview
The Aged Care Connector (ACC) has the primary responsibility of connecting with local Elders, older Aboriginal and Torres Strait Islander people and their families to raise awareness of their Aged Care entitlements.
The Aged Care Connector will work closely with the Aged Care Support Coordinator. They will also help determine the level of local demand for Aged Care services, identify existing Aged Care services, any service gaps in the area and continue advocacy work with clients as required.

Requirements:
- Excellent communication and interpersonal skills, including an ability to communicate sensitively and effectively with patients with a caring attitude.
- Ability to multi-task and work well under pressure.
- Sound computer skills and experience with MMEX or another clinical software package.
- Well presented friendly, organised and reliable, with accuracy and excellent attention to detail.
- You must have an Open C-Class driver licence valid in Queensland.
- Satisfactory Police Check – no Serious/Criminal/Court Record and ability to obtain a Blue Card. This will be conducted on initial appointment and on an annual basis in line with Carbal’s compliance schedule.
Essential
- Certificate III in Individual Support for aging and disability.
- Certificate in Community Services and/or Aboriginal Mental Health First Aid will be highly regarded.
- Knowledge of the Aged Care System, however training will be provided.
- Demonstrated understanding of common health issues affecting Aboriginal and Torres Strait Islander communities.
