Job description

Core tasks
•Undertake the frontline responsibilities of staff health and safety as per policy
•Maintain accurate and up to date documentation including notes, assessments and care plans
•Attend shift handovers to ensure accurate transfer of clinical information
•Complete assessments and the monthly Health & Wellbeing checks.
•Observe, report and document all changes in resident need requirements
•Oversee care staff in ensuring maintenance of clean linen, and making of beds for residents
•Oversee care staff assisting with the serving of meals assisting residents as required
•Oversee the delivery of specialised nursing needs for residents using contemporary practice
•Provide consistently high quality care to all residents
•Support independence of residents by providing appropriate assistance with personal care in accordance with care plans
•Undertake delivery of medication to residents according to policy and procedure
•Mentor, coach and supervise other care staff
•Provide assistance to doctors as required
•Ensure assessment of new residents is completed and added to Leecare in a timely manner
•Undertake medication management including ensuring medication competencies are conducted, medication errors are reported and medication is delivered to residents according to policy and procedure
Client services
•Liaise with staff, residents, visitors, GPs and allied health professionals to provide prompt and friendly service
•Act to protect the rights of residents including confidentiality, privacy, individual choice and decision making
•Provide emotional support to residents

•Build and maintain effective relationships

•Complete annual medication competency within the required timeframes
•Ensure compliance against the Aged Care Standards is maintained where appropriate
•Be responsive to the needs of PCAC and its changing environment

•Attend meetings and compulsory education as required
•Identify self-education needs

Skill requirements
Essential
•Demonstrated excellent communication and interpersonal skills
•Must be a team player with the ability to multi task
•The ability to demonstrate a professional and friendly manner at all times
•Demonstrated high level of analytical, written and verbal skills
•Demonstrated initiative and the ability to work without supervision
•Demonstrated excellent time management skills
•Demonstrated ability to supervise and direct staff in providing care
Desirable
•Mature outlook with the ability to relate and converse with older people
Knowledge requirements
Desirable
•Working knowledge of Aged Care Standards and legislation
Experience and/or qualifications
Essential
•Current registration with the Victorian Nurse’s Board as a Division 1 Registered Nurse
•All PCAC appointments are subject to a satisfactory police check with no disclosures
•Current Victorian driver’s license
Desirable
•Experience in an Aged Care setting
Physical requirements for the role
Essential
•Demonstrated strength and physical fitness appropriate to the role such as movement of stock and operation of trolleys and resident lifting equipment
