About the role

The Case Manager role is responsible for providing high-quality client services through assessment by visiting clients in their homes and developing support plans and referrals, planning, coordinating and implementing services, ongoing monitoring of client satisfaction and quality of services. This position is a permanent Full-Time role, Monday to Friday, 76 hours a fortnight.

About You

To be successful for this role, you must have a Diploma in Case Management or Community Services or equivalent qualifications / previous experience in case management and care coordination. Clinical experience preferred, but not necessary.

As well as:

  • Previous experience or understanding of contemporary case management practice, Home Care Standards, the Aged Care Act, Consumer Directed Care and Home Care Guidelines;
  • Excellent interpersonal and communication skills, with the ability to liaise and build a rapport with a range of clients; 
  • Exceptional time management skills, with the ability to manage multiple competing priorities and deadlines without compromising quality;
  • Strong problem-solving skills, with a solution-focused style and approach and the ability to think outside the square;
  • Sound administrative and computer skills; 
  • Current driver’s licence, together with a safe driving record; 
  • Use of a reliable, well-presented vehicle that has fully comprehensive insurance coverage; and
  • Evidence of COVID-19 Vaccinations (primary vaccination course, plus one booster).
  • Able to provide evidence of protection against the infectious diseases listed in the relevant policies of NSW Health relating to the OHC program: Diphtheria, Measles, Hepatitis B, Pertussis, Mumps, Varicella, Tetanus, Rubella, Tuberculosis, Influenza.

The successful candidate must provide a National Criminal History Check. The criminal offences that preclude a person from working in aged care are; a conviction for murder or sexual assault or a conviction of, and sentenced to imprisonment for, any other form of assault.
