About the Role:We are seeking a motivated
APS4 Planner to join our clients team. This role plays a critical part in ensuring the effective delivery of services to participants within defined parameters. As an
APS4 Planner, you will apply established policies, principles, and procedures to support the organisations objectives, contributing to the growth of the organisation.
Key Responsibilities:- Gather information from participants and planning partners through effective communication channels, ensuring high-quality planning and evidence-based decision-making.
- Utilise comprehensive knowledge of policies, practice guidance, and legislation to make informed funding decisions for participant plans, integrating informal, community, and mainstream supports as needed.
- Collaborate closely with key stakeholders, including Partners in the Community, to coordinate and deliver services that meet participant needs.
- Resolve issues and address complaints related to participant plans promptly and effectively.
- Complete administrative duties and maintain accurate data entry regarding participant records and plans.
- Contribute actively to achieving key performance indicators, including planning targets, to enhance service delivery effectiveness.
Requirements:
- Proven experience in a similar operational or administrative role, preferably within the disability sector or related fields.
- Sound understanding of policy implementation, with the ability to apply policies, principles, and procedures effectively.
- Strong communication and interpersonal skills, capable of engaging with a diverse range of internal and external stakeholders.
- Ability to analyse information, conduct research as required, and make informed decisions that support participant outcomes.
- Demonstrated organisational skills and attention to detail, ensuring accuracy in administrative tasks and data management.
- Commitment to contributing positively to team goals and achieving key performance indicators.
