***Mandatory: Australian Citizenship required to work within Federal Government***
Our Federal Government client is seeking to engage in a Multiple APS6 Program Support Officers for a 12-month assignment with potential extension in ADELAIDE CBD
About the team
YPIRAC Hospital Interface Branch is responsible for servicing two cohorts of participants; those living in or at risk of entering aged care under 65 years of age, and those in hospital who are medically ready for discharge but have barriers to doing so. The branch has a specialised pathway for these participants which align to Ministerial priorities with the view to expediting quality outcomes.
YPIRAC team works to prevent younger people who are eligible for the NDIS from entering residential aged care, or to leave residential aged care if this is their goal. The residential aged care system is designed to support the needs of older people. It is not designed for younger people. The Agency supports younger participants to have more choice and control over where and how they want to live.
The Hospital Interface team helps participants discharge from hospitals, the team has specialised hospital discharge planners and health liaison officers who prioritise participants’ plans in hospitals to help them discharge in a safe and timely way. This helps participants avoid staying in hospitals when they are medically ready for discharge.
