The successful applicant will be an experienced RAS or ACAS assessor who will be required to primarily undertake face to face home support assessments, also support plan reviews and some assessments over the phone, The Regional Assessment Officer (RAO) will be required to match and refer clients to appropriate services as required. The RAO will work with older people and their carers across the Local Government catchment areas. The RAO will possess the ability to assess their client's care requirements in the context of their psychosocial and functional needs.
What you bring:
- Experience undertaking home and phone based holistic assessments as a registered Division 1 Nurse.
- APHRA Registration
- Previous experience within My Aged Care (MAC) system/portal or in a CHSP/HACC (or other funding) environment and/or familiarity with the community services network.
- Demonstrated experience in working with older people and/or persons with a disability, in areas of assessment, care planning and care co-ordination.
- Ability to engage clients from diverse backgrounds and establish rapport.
- Strong IT skills including Microsoft Office suite.
- Ability to work autonomously including being well organised and highly developed time management and prioritisation skills.
- Current Victorian Driver’s Licence.
- Knowledge of Occupational Health and Safety principles and systems.
