About the role
As a member of the Community Respite team, the Buddy Program Coordinator (EN) is responsible for the high level clinical oversight, assessments, care planning and case management of Buddy Program clients living with dementia and other complex health conditions.
The Buddy Program Coordinator will provide leadership and clinical advice to Buddy Support Workers to ensure best practice service and care is planned, aligned and delivered with consideration to a clients competency, capability and dignity of risk.
Key Responsibilities include but are not limited to:
- Completing proficient, person-centred clinical & non clinical assessments, scheduled and urgent care plan reviews
- Monitoring and evaluating each Buddy Program service to ensure the delivery of a high quality respite service model that focuses on the reablement and wellness of clients
- Monitoring and responding to client deterioration and incidents, identifying early signs of decline to provide best practice clinical intervention for the client
- Educating and supporting Buddy Support Workers to find creative solutions to overcome barriers to client participation in meaningful activities in order to help achieve their well-being goals
- Partnering with internal and external stakeholders to facilitate referrals into and out of the Community Respite Programs

About you
We are looking for people who will be the difference in the lives of our residents and clients.
The successful candidate will have the following skills and experience:
- Diploma of Nursing with current registration with AHPRA as an Enrolled Nurse.
- At least 5 years of Dementia Care experience
- Experience in aged care health and community services, clinical case management and collaboration
- Exceptional interpersonal, negotiation and communication skills
- Ability to provide leadership in the management of community aged care staff and services to a range of client groups and working with multi-disciplinary teams and health professionals
- Demonstrated project management, analytical and problem solving skills
- Proficient computer and keyboard skills and the ability to use Google Suite and various in-house databases.
- Demonstrated experience working as an Enrolled Nurse in a Community or Residential Aged Care setting will be desirable
- Current Driver's Licence (Class car) - including willingness and ability to travel in-between our various Respite sites
