We are looking for a passionate health professional who is looking to grow and further develop their career in Chronic Disease Management with an organisation dedicated to giving back to the community.
The purpose of the role is to expand the scope and reach of the Chronic Disease Management (CDM) service to ensure that the community have improved access to assistance with the management of their condition; primary conditions include respiratory, diabetes and cardiovascular disease.
The role will work closely with the CDM team including CDM team leader on a day to day basis to focus on outreach engagement and achieving positive clinical outcomes of community members
To be successful, you will require;
- Tertiary qualification in Nursing or Allied Health or a Certificate IV or Diploma of Nursing
- Current registration with Australian Heath Practitioner Regulation Agency (AHPRA) (If applicable).
- A current Working with Children Check
- Current Victorian Drivers License
