About the role:
The HARP-Disease Management Team (HARP-DMT) are seeking an experienced Enrolled Nurse to join their team 4 days a week on a permanent basis. The Disease Management Team home visit clients post discharge from hospital to provide education and support in the management of COPD and CHF.
The role of the Enrolled Nurse in HARP-DMT is to support the team in assisting clients to improve their health and self-management of their condition. This involves conducting home visits to supervise home exercise programs, and supervising community exercise groups as well as the provision of education relevant to their health condition.
Applicants must be self-motivated and possess sound organisational, time management, and clinical skills. Interest and/or further education in chronic disease or exercise prescription is desirable but not essential.
The successful applicant will have:
- Current registration with the Australian Health Practitioner Regulation Agency as an Enrolled Nurse
- Demonstrated knowledge and experience working with people who have chronic illness
- High level of clinical competency
- Self-motivated, with high levels of communication, organisation, and time management skills
- Intermediate level of computer literacy
- The ability to work independently and with autonomy when required
- Able to work effectively as part of a multidisciplinary team
