Clinical Nurse Coordinator - Palliative Care

Based in Gippsland, Latrobe Regional Health are looking for a Clinical Coordinator to join the Palliative Care Consultancy Gippsland (PCCG) on a permanent, part time basis.
The Palliative Care Consultancy Gippsland has been established to deliver specialised Palliative Care across Gippsland. The service aims to provide specialised Palliative Care to patients and their families according to their preferences and goals.
The Palliative Care Coordinator is responsible for delivery of expert clinical guidance, discharge coordination, advice and clinical leadership for the target population in the LRH inpatient setting and across the sub-regions of Gippsland.

We are looking for candidates with:
- Registered Nurse with clinical experience in Palliative Care / chronic disease management
- Knowledge of the concepts and practice of Palliative Care experience in advising, supporting and educating team members in these concepts
- Excellent interpersonal skills and the ability to work well as part of a team
