Who are we looking for?
We are seeking an inquisitive and dynamic individual to oversee and provide exceptional person-centred care management services for our clients who are recipients of home care packages.
You will prioritise practice development and want to take a holistic approach which focuses on supporting our clients to achieve health and well-being in their day-to-day activities.
You will genuinely enjoy working with older people and won’t hesitate to find ways to support their independence. To help you succeed in the role, you will have a solid understanding of the Aged Care Quality Standards and want to continually strive to find ways for yourself and for us to grow.
Primary Responsibilities
- Delivery of care management services to home care package recipients.
- Contribute to strategic and operational planning to support client-centred service delivery.
- Work collaboratively with Care Companions (support workers) to ensure appropriate delivery of in-home care services.
- Build meaningful and respectful relationships with clients in a context of empowerment and applying creative responses to addressing needs.
- Identify risk factors, engage in a co-management approach to care planning and ensuring appropriate referral of clients to specialised services.
- Prioritise the integrity of client related documentation and data entry, maintaining accurate and timely records in Age Up Health’s client care management systems
- Maintain effective collaborative working relationships with key stakeholders to ensure sound planning and delivery of services for home care package recipients
- Supervise and allocate tasks to Care Support Specialists in accordance with Age Up Health policies and procedures.
You will have:
- Case management experience in the aged care or similar industry (Nursing (Div 1 or Div 2) or equivalent favourable)
- High-level oral and written communication skills, including the ability to write care plans, complete reports, and other professional documents within strict timeframes.
- Passion and commitment to providing high-quality services to home care package recipients.
- Well-developed skills in building purposeful relationships with clients to support them in achieving their goals
- Excellent interpersonal skills with the ability to work supportively and collaboratively across teams.
- Resilience and tenacity to work in a fast-paced work environment.
- Australian Driver’s Licence and well-maintained Car.
