As a Care Manager, you will work with us 3 days per week to:
- Manage client service delivery across the Short-Term Restorative Care (STRC) Program.
- Develop care plans aligned with the client’s needs and goals in conjunction with the Aged Care Assessment Team (ACAT) assessment.
- Undertake marketing and collaborate with external allied health providers, medical professionals, and specialist care providers.
- Co-ordinate multidisciplinary teams to deliver a best care approach.
- Utilise your business development skills to drive the growth and utilisation of the Short-Term Restorative (STRC) Programme.
- Ensure compliance with all relevant legislative and industry standards.
- Deliver excellent client care for elderly Australians.

We are looking for someone with:
- AHPRA registered allied health discipline or RN registration also considered
- Experience in aged care, with knowledge of the Short-Term Restorative (STRC) Care Programme
- Frontline management skills
- Current First Aid and CPR certificates
- A current Australian drivers’ licence, vehicle registration, and access to your own car
- National Police Clearance or willingness to obtain (myHomecare will organise via WorkPro at no cost)
