Key Purpose
To provide support and assistance to consumers as a partner in the coordination and management of consumers individualised Home Care Package or Short-Term Restorative Care Program.
Core Functions
- Coordinating Home Care Packages and Short-Term Restorative Care Packages as required.
- Coordinates and facilitates the multi-disciplinary team for the coordination of Short-Term Restorative Care Packages as required.
- Assists in the Coordinating the Home Independence Program, as required.
- Service intake, Coordination of home care and support, consumer agreements, support planning, brokerage of services, referral, and care-management, including but not limited to:
- Identification of consumer's needs and goals.
- Development of consumer directed goal-based support plans incorporating a wellness approach.
- Service contracting and brokerage of services as appropriate.
- Facilitating the introduction of support and/or care workers to the consumers
- Monitoring of services that are provided to ensure consumers' needs and goals are being met.
- Contributing to a multidisciplinary team approach.
- Liaising with referral agencies, service providers, consumers, family/advocates to ensure best possible outcomes for consumers.
- In partnership with the consumer monitoring and reviewing their progress.
- Accurate and timely documentation and maintenance of all Consumer records.
Essential Requirements
Registered Nurse with current unrestricted AHPRA registration.
Current unrestricted Tasmanian Drivers Licence.
National Police Certificate obtained within the last three (3) months.
Covid -19 Vaccination x 3 doses.
