Care Forward is a locally owned and operated organisation, specialising in health and homecare services designed to support the independence of older Tasmanians and people living with chronic conditions or disabilities.
For 25 years we’ve been providing care and support to people in their homes across Tasmania. Our dedicated and passionate team includes nurses, allied health professionals and home independence support staff – all working to support and restore independence, health and wellbeing to thousands of Tasmanians across the State.
Location of Role
The role is located in our Burnie office.
Employment Status
Permanent / Full Time or Part Time but a preference for Full Time.
About You
You are an enthusiastic and dedicated Professional with a real passion for improving the lives of our consumers and supporting a team of health professionals to deliver quality consumer care and support. With the ability to work autonomously and lead your team, you will thrive in this multidisciplinary environment. You are adaptable, enjoy meeting new people and working in a variety of settings.
Key Purpose
To provide support and assistance to consumers as a partner in the coordination and management of care and services within the Home Care Package Program (HCP).
Core Functions
- Coordinating all elements of the consumers HCP.
- Service intake, Coordination of Home Care and support, consumer agreements, support planning, brokerage of services, referral, and care-management, including but not limited to:
- Identification of consumer's needs and goals.
- Development of consumer directed goal-based support plans incorporating a wellness approach.
- Coordinating appropriate care and services to ensure best possible outcomes for consumers.
- Regular monitoring and review of services to ensure goals are being met.
- Contributing to a multidisciplinary team approach.
- In partnership with consumer, manage the HCP budget allocation.
- Accurate and timely documentation and maintenance of all Consumer records.
Essential Requirements
- Registered or Enrolled Nurse.
- Five years’ experience working in an aged , health or community care setting.
- High level verbal, written and interpersonal communication/relational skills.
- Current unrestricted Tasmanian Drivers Licence.
- National Police Certificate obtained within the last three (3) months.
- Covid -19 Vaccination x 3 doses.
- Two (2) professional referees, preferably from most recent employment or educational institution.
