12th January, 2024 About the role The goal of the Team Leader is to support the Case Management Team to deliver high quality individual service to each participant on the ALPA caseload. It will also support the CDP participants to improve their quality of life and access the opportunities available within the community. It is important that the Team Leader is focused on the development of local community members and delivering a high-quality service to them, while representing ALPAThis position is based in Gove, Nhulunbuy in the Northern Territory and will require for you to relocate here. Responsibilities Provide leadership to the Case Management Team, ensuring that all program guidelines are met and provision of high-level reporting is provided to the Operations Manager. Provide administration support to the site including logistics, rostering, and meeting minutes. Build and maintain relationships with the CDP team, existing employers, potential employees and community members to promote the benefits of the CDP. Utilise and understand the Employment and Community Services Network (ECSN CDP IT) to undertake Case Management. In Case Management of individual participants identify barriers to participation, identify solutions and work with participants to overcome barriers including the provision of administrative support. Measure, record and document in the manner and timing determined by ALPA the performance measures established for the CDP program and funding agreement.
