About this job: The Continuing Care Case Manager Support is to provide culturally appropriate, ongoing professional support to individuals who reside at FORWAARD Aboriginal Corporation’s Transitional House or who are external clients of our Continuing Care Program. In this program, you will support clients in integrating back into the community through work, education, and training or as a volunteer. This will require you to work with our clients as groups or individuals, support participation in activities, implement weekly routines, and document each activity as needed.
