Founded in 2011, Melbar was established to serve the elderly and vulnerable individuals within the Chinese community. After years of significant and sustained growth, Melbar has expanded their operations to serve the broader Asian and Southeast Asian communities through in-home Aged Care and community assistance services.
With a client base of over 1,000 individuals, Melbar is seeking a new Case Manager that can join a growing case Management team.
Reporting to the Senior Case Manager, the Case Manager will enhance the quality of client management, maximise satisfaction, and promotes cost effectiveness by delivering services to the ACQS. The role will be accountable for assigned clients and is expected to assess, plan, implement, monitor and evaluate actions required to meet the client’s health and human services needs.

This role will hold key accountabilities around the following:
- Build positive and trusting long-term relationships with clients, customers, allied health professionals, and care workers
- Work collaboratively with the operations team to support clients with their specific needs and goals, contributing to the vision of fostering independence, wellbeing, and peace of mind
- Ensure best clinical case management practices are used to align care plans with client needs and deliver effective services within those parameters
- Monitoring client progress and ensuring case plans are updated as needed.
- Coordinating with other healthcare providers to ensure a continuity of care for clients.
- Facilitating communication between clients, families, and other healthcare providers to ensure seamless care.
- Advocating for clients and ensuring they receive the necessary services and support.
- Assisting clients in navigating the healthcare system and accessing community resources.
- Monitoring clients' utilization of their home care package funding and ensuring it is used effectively and efficiently.
- Relevant qualifications in Care with Community experience e.g. Certificate 4 in Aged Care and/or Home and Community Services, Care management or Nursing.
- Knowledge of the Home Care Packages system.
- Mandarin/Cantonese speaking required.
