Position Overview: As a Case Manager at TDN, you will play a pivotal role in coordinating services for a diverse range of clients and their families or carers. Your primary objective will be to support recipients of Home Care Package funding who aspire to maintain their independence and live comfortably in their own homes. You will be working from our satellite office located in Launceston.
This position attracts the pay scale range of Level 3 Pay point 1 ($42.23) to Level 3 Pay Point 5 ($48.05) based on your qualifications and experience, within our enterprise agreement.
What We're Looking For: We are seeking a dedicated, client-focused individual who possesses the necessary experience, knowledge, and skills. You should be energetic, have the ability to collaborate with various healthcare providers and team members.
Key Responsibilities:
- Conduct integrated assessments to understand the unique needs of each client.
- Collaborate with a variety of healthcare professionals and service providers to coordinate an array of services.
- Establish and maintain strong relationships with key stakeholders involved in the client's services.
- Ensure that government funds are efficiently utilized within the bounds of legislation and guidelines.
Key Requirements:
- Exceptional organisational skills
- Commitment to working within a multidisciplinary team
- Proficiency in verbal and written communication
- Strong computer skills, particularly with Microsoft Suite and client management systems
- Able to self-manage and work independently
