THE POSITION:

Stream Healthcare is recruiting Case Manager in Regional WA! We are looking for experienced Case Manager that have an interest in relocating, living and working in regional WA whilst assisting families. The positions will be working closely with the community to advocate and provide the best possible outcomes for service users and their families.

SELECTION CRITERIA

• Fully qualified Social Worker, AHPRA Mental Health Nurse or AHPRA Psychologist 
• Experience within mental health is essential 
• The ability to obtain a National Police Clearance and Working with Children Check
