Alpine Care Group is a leading provider of support services within the NDIS sector. Our mission is to empower individuals with disabilities to live fulfilling and independent lives. We operate all around Perth, including Perth Metro, Joondalup, Fremantle, Midland, Armadale, and mainly South the river from Baldivis to Mandurah. We offer a range of services, including core supports, SIL, STA, MTA, and Community Access Supports, tailored to meet the unique needs and goals of each participant.

Job Description:

We are seeking a dynamic and experienced Specialist Support Coordinator/ Case Manager to join our team in Perth. The ideal candidate will have a strong understanding of the NDIS and proven networking experience within the sector. You will be responsible for coordinating and implementing support plans for NDIS participants, as well as building relationships with key stakeholders to drive referrals to Alpine Care Group.

Key Responsibilities:

· Coordinate and implement individualized support plans for NDIS participants.

· Maintain strong relationships with Local Area Coordinators, allied health professionals, and Support Coordinators to provide referrals to Alpine Care Group.

· Monitor and assess participant progress and adjust plans as needed to achieve goals.

· Advocate for participants' needs and rights within the NDIS framework.

· Provide participants with information and support to make informed decisions about their care.

· Collaborate with internal teams and external providers to deliver high-quality services.

Building a team of SC as this would be a blank canvas and new department in Alpine Care Group.

Requirements:

· Minimum of 1 year of experience as a Specialist Support Coordinator/ Case Manager within the NDIS sector.

· Strong networking skills and established relationships with Local Area Coordinators, allied health professionals, and Support Coordinators in Perth.

· Valid visa allowing the candidate to work in Australia.

· Excellent communication and interpersonal skills.

· Ability to work independently and as part of a team.

· Strong organizational and time management skills.

· A valid driver's license and access to a reliable vehicle.

Additional Employment Requirements:

· National Police Clearance.

· NDIS Screening Check.

· Working With Children Check.

· First Aid Certificate.

· A relevant tertiary qualification is highly desirable such as Occupational Therapy, Social Work or Developmental Educator or any relevant experience, degrees, and qualifications.
