Key Responsibilities:
- Conduct comprehensive assessments of clients' needs to facilitate coordination of supports and aligning with clients’ goals, considering physical, emotional, and social factors.
- Develop individualized care plans in collaboration with clients, their families, and healthcare professionals.
- Act as a central point of contact, coordinating and liaising with clients, their families, healthcare providers, social services, and community resources.
- Foster effective communication among interdisciplinary teams including our support workers to ensure holistic and seamless care delivery.
- Maintain accurate and up-to-date client records, documenting assessments, care plans, and progress notes.
- Provide support to the Senior Case Manager to facilitate effective and comprehensive service delivery for our more complex clients.
- Prepare detailed reports for internal and external stakeholders as required.
- Advocate for clients' rights and needs within the healthcare system.
- Provide emotional support and guidance to clients and their families throughout the care process.
You will be supported by a senior Case Manager, along with the broader admin team in our fast-paced environment, supporting and driving meaningful Aged Care service delivery, as well as liaising with clients and our partners and stakeholders. You will be a key part of our small team in a growing organisation.
To successfully fill this role, you will have:
- A degree in Nursing with full and current registration with AHPRA is essential; and proven experience in Aged Care is highly desirable.
- Strong interpersonal and communication skills.
- Ability to work collaboratively in a team and independently.
- Empathy, compassion, and a commitment to upholding the highest standards of care.
- Outstanding problem solving and exceptional time management skills thriving in a face paced environment.
- Ability to establish respectful relationships with clients and support workers.
- Strong attention to detail and a high-level of proficiency in written and verbal communication skills.
- Microsoft Suite expertise is preferred.
