Due to a restructure, internal promotion and growth, Home Instead are again expanding our wonderful Client Care Team and look forward to hearing from prospective Care Managers! We have the tools to upskill, develop and support and we are excited to have a new part-time Care Manager join us, flourish and drive client goals and outcomes.
A Care Manager is responsible for successfully directing and coordinating all activities associated with the care of potential and existing clients. Building positive, trusting relationships is pivotal to the success of this role. Providing excellent customer service is essential; you must be hardworking, resilient, positive, customer focused and a great communicator.
A day in the life of a Care Manager may see you visiting prospective clients, reviewing existing clients, identifying needs, understanding goals and creating robust, personalised care plans and actions. As a Care Manager of Home Instead you have a dedicated Client Services Specialists who supports you to coordinate the CAREGiver teams and a dedicated Care Coordinator to assist you with administrative functions, diary management, purchases, quotes and the paperwork maze that can be Care Management!

Skills & Experience
To be considered for this role you must have/be:
- Case Management experience, specifically in HCPs, will be highly regarded.
- Either have or willingness to obtain a relevant qualification in Aged Care, Disability, Individual Support, Nursing, Community Services, Social Work, Allied Health (If you don’t have a minimum of a Cert IV Community Services we will schedule you to complete whilst working - paid study leave provided)
- Efficient and organised.
- Able to prioritize effectively and have good time management skills.
- An effective communicator.
- Customer service centric and focussed on the Client Experience
- A valid Driver's Licence and your own car
- A clear Police Check
- An effective delegator and a team player
- A positive mindset and appreciate the importance of your role
- Understand the importance of documentation and be savvy in your note taking, care planning and data entry.
- Experience with MS Office and CRM systems.
- Knowledge of Aged Care and Home Care legislation or the willingness to learn.
