About the role:
The role of Case Manager in Residential Care is to service high-risk young people in Protective Care and or Leaving Care. The role will ensure each young person receives comprehensive case management and support. They will also ensure effective planning and coordination of services for reach young person. They will be required to develop, implement, monitor and review a variety of plans for each young person that matches their needs all within the parameters of their Child Protection case plan.
- Full time (76 hours per fortnight)
- Ongoing (subject to funding)
- Inner & Outer Gippsland Location
Selection Criteria:
- A relevant tertiary qualification such as a degree in Social Work, and one year’s experience in theprovision of support to young persons with complex needs and challenging high risk behaviours.
- An understanding of the developmental and therapeutic needs of young people who haveexperienced trauma, abuse and neglect.
- Knowledge of a range of theoretical perspectives with the ability to apply these theories whenworking with young person’s on statutory orders.
- An ability to work within a case management framework, including the ability to adequatelyassess young people’s needs and develop, evaluate, and vary plans in relation to those needs.
- Highly developed communication skills and organisational ability.
