Form of workIf you are an experienced community services Case Manager, this is a great opportunity!
Our client is a leading provider of community services, with a strong footprint across NSW and interstate. They are in need of an experienced person to join their team on a contract basis, leading the provision of support to families and individuals within the Housing, Child, Youth and Family services space.
As a Case Manager, you will work closely with clients to build positive relationships and deliver comprehensive support which reduces risks and creates positive change for individuals and families.
Your role will include:
- Case management of clients who are experiencing complex issues; conducting client interviews, identifying and addressing their needs, performing home visits, and managing referrals and advocacy
- Managing all facets of case planning, safety planning, support and mentoring for individual clients, families, children and young people, whilst working collaboratively with external services to deliver holistic support
- Collaborating with and advocating for the needs for Aboriginal people and their extended families, including development, implementation and regular review of cultural case plans
- Performing associated administration and reporting tasks, as required
In order to succeed in this role, you will need:
- A Degree or Diploma in Community Services, or similar/equivalent
- Experience working with Aboriginal people, communities and/or organisations
- Outstanding communication, interpersonal and advocacy skills
- Excellent administration and teamwork skills
- Understanding of legislation and reporting requirements for child protection
- Experience working with people at risk of or experiencing homelessness is desirable, but not essential
- Working With Children Check
- Current Driver’s Licence
