Case Manager - Supported Independent Living - Permanency Support Program
- Leave coverage: Full-time max-term contract commencing ASAP until 21/06/24

- This role is paid under the Social, Community, Home care & Disability Services Award 2010 - Level 5 $47.50 - $49.65/hour
The Opportunity
The Case Manager is responsible for all aspects of program delivery to ensure the safety and wellbeing of young people in the program. They proactively engage with young people, through regular home visits, professional and accountable casework, advocacy and planning. Case Managers are required to create social relationships with family, significant others and friends, to apply Family Finding program for connections /reunions which are supported and monitored with the young person.

Essential Criteria
- Tertiary degree qualification in Social Work, Psychology, Social Science, Early Education or similar. Applicants who are working towards this qualification will be considered if they have minimum of three years’ experience in a similar role.
- Demonstrated ability to use a holistic based approach in implementing and overseeing case management, case plans that incorporate appropriate models of intervention, create opportunities, empower and promote the wellbeing of the child /young people through engagement of families and carers
- Strong interpersonal skills that provide the basis for effective negotiation, mediation and conflict resolution practices and outcomes
- A non-judgemental and empathetic approach to engaging and working with children and young people and their families
- Demonstrated understanding of the complex needs of young people and families/careers facing leaving care.
- Highly developed case management documentation skills including computer literacy
- Proven ability to develop rapport with carers, child /young people and their families/significant others in order to support informed decision-making and skills development for living within the community
- Demonstrated commitment and ability to maintain confidentiality and accountability, including records of children and young people and their families, team, and organisational discussions.
- The ability to effectively develop and maintain positive working relationships and networks with other Government and Non-Government agencies/services
- Demonstrated ability to participate positively in teamwork, training, and supervision processes.
- Demonstrated understanding of the relevant Government legislation applicable to the Supported Independent Living Program
