👉🏼 The need-to-know…

As Rise’s new Case WorkerYouth Justice, you will be responsible for the delivery of the Hope Agreement. In this role, you will assist young people to address complex issues that are negatively impacting on their life skills, health, and development. This assistance includes, but is not limited to, mentoring, and supporting them to access medical and other health services, such as registering with a GP, dentist, or psychologist, facilitating referrals to other services, and increasing leisure and recreational opportunities for the young person. Additionally, you will collaborate with other consortium partners by attending meetings, sharing good practice, collating data, reporting upon outcomes, and promoting the service. 

A typical day might look like….

  • Maintain a collaborative relationship with Hope and other consortium partners.
  • Establish and maintain links with relevant community-based organisations.
  • Monitor the referral/support process with relevant community stakeholders.
  • Provide case management to the young people in line with the service guidelines.
  • Monitor and review the young people’s support plans on an ongoing basis.
  • Resolve complaints and action compliments promptly and in line with organisational requirements.
  • Maintain accurate records/documentation including client case notes.
  • Monitor compliance with program policies and procedures.
  • Contribute to the monthly Divisional program reporting requirements.
  • Provide timely information for internal and external reports.

This role is part-time, working 19 hours per week over 3 days a week. Flexibility around working days can be discussed. This role is community-based working across the north & central Perth Metro areas, with some visits to Banksia Hill detention centre. 

👉🏼 The need-to-have…

Here at Rise we nurture potential, but we do need you to have a few tricks up your sleeve including the following: 

  • Relevant qualifications in Community Services and/or working towards the qualification and/or relevant experience in a related role.
  • Well-developed communication and interpersonal skills evidenced by your experience of working with young people experiencing complex issues, their families, friends, advocates, and others.
  • Demonstrated knowledge and understanding of the issues impacting on young people at risk.
  • Ability to organise youth events and group activities.
  • Ability to work with minimal supervision in a community setting. 
  • The ability to manage and prioritise your own workload. 
  • Previous computing experience and average level knowledge of common computer programs such as internet and word processing programs.

Appointments to this position will be subject to a NDIS Worker Screening Check / Working with Children Check / Youth Justice Services Program Screening Check.

Costs for new checks will be covered and/or reimbursed by Rise.
