About Centrecare
Centrecare is a not-for-profit organisation with just over 300 employees and is committed to delivering quality professional counselling, support, accommodation, mediation and training services in over 11 locations across the Perth metropolitan, Goldfields and South West regions.
The Role
This position is part of the ReSet consortium led by Wungening Aboriginal Corporation, providing support to men and women in prison to have new beginnings and break the cycle of offending. Working as part of a team, this role is responsible for delivering comprehensive throughcare support services, including integrating evidence-based practices for offender rehabilitation and reintegration, aligned with the Department of Justice’s Individualised and Integrated Offender Management (IIOM).
The role is a full-time position based in our Gosnells office, with frequent visits to Perth metropolitan prisons and community outreach.
A few specifics:
To provide a service to individuals reintegrating into the community from custodial settings, with a focus on:
- Conduct client assessments to develop case plans and referrals based on identified needs.
- Implement case management and counselling techniques to support offenders' reintegration.
- Maintain documentation including case notes, assessments, and support plans.
- Foster collaborative relationships with internal and external stakeholders.
- Provide family and domestic violence risk assessment, safety planning, and risk management.
- Support individuals in crisis through specialist referrals and tailored throughcare programs.
What we are looking for
- Tertiary degree, Diploma in Community Services, Counselling, Criminology and Justice, social work or other related discipline
- Demonstrated experience working in a Justice Setting or similar field, with clinical, case management and counselling experience.
- Demonstrated ability to work with Aboriginal and Torres Strait Islander families and children in contact with the justice system.
- Ability to network, negotiate and liaise with external agencies and government agencies.
- Demonstrated ability to be part of a team, contributing towards culturally responsive, quality service delivery and positive client outcomes.
- Ability to obtain a National Criminal History Record Check and Department of Justice Integrity Check.
- Possession of a current 'C' class driver's license and vehicle.
- National Police Clearance and Working with Children Check.
