Applications Close: 16 April 2024What you'll be doingThe Peer Supported Transfer of Care (Peer STOC) initiative will provide assertive support and post discharge follow up consumers who are transitioning between acute Mental Health care and the community.The Peer STOC initiative will work to actively reduce the 28 day readmission rate for consumers leaving acute Mental Health care by providing peer support for up to six (6) weeks following discharge.The Peer-STOC initiative offers hope to consumers who are leaving acute Mental Health care, facilitates contact with people who have a personal lived experience of recovery and firsthand experience in wellbeing management.Peer-STOC workers will provide assertive in-reach for consumers in acute Mental Health units for 7 days prior to their discharge. These meetings are to establish relationships, build rapport, discuss wellbeing strategies and collaboratively identify supports needed to transition into the community.Peer-STOC workers will provide direct face-to-face follow up on the day directly following discharge, and no later than 72hours after discharge. Support will be provided for up to six (6) weeks following an acute admission.Peer –STOC pe4er workers will deliver support that aligns with the principals of Step up/Step Down care. This includes making referrals to a range of community based services for ongoing support if required (Step Down), or escalating concerns ore consumer wellbeing to clinical staff within the Community Mental Health team they are located in for assertive follow up to support hospital avoidance. (Step Up)Priority for referral should be given to consumers who have no formal identified community supports at the point of discharge. Existing support includes Care Coordination through Community Mental Health Teams, HASI, HASI Plus, PHaMs, PIR, NDIS etc.All NSW Health workers are required to have completed an up-to-date course of a COVID-19 vaccine (2 doses) which has been approved or recognised by the Therapeutics Goods Administration (TGA). New applicants must have completed the vaccination course prior to commencement with NSW Health, or provide an approved medical contraindication certificate certifying the worker cannot have any approved COVID-19 vaccines available in NSW.Acceptable proof of vaccination is the Australian Immunisation Register (AIR) Immunisation History Statement or AIR COVID-19 Digital Certificate. • Minimum Qualifications: Certificate IV in Peer Work (or enrolled in Certificate IV Peer Work).
• Demonstrated effective interpersonal, written and verbal communication skills.
• Demonstrated commitment to the delivery of recovery orientated Mental Health service delivery.
• Demonstrated ability to work with minimal supervision and as part of a multidisciplinary team.
• Demonstrated computer literacy skills and knowledge of Microsoft Office and health applications.
• Current NSW Driver’s License (Class C- unrestricted).
